Health Care Decision-Making A Georgia advance directive is a process for an individual to appoint another person to make decisions on their behalf.  A Georgia advance directive takes into consideration factors including: 1) Ability to make health care decisions 2) Health status 3) Personal and financial concerns 4) Willingness of the patient to receive medical treatment 5) Willingness to participate in health promotion activities or participate in a patient education outreach program 6) Financial status 7) Personal preferences 8) Medical condition 9) Social ties 10) Communication 11) Location of patient (i.e., home, private space, hospital, nursing home) 12) Spouse's consent These factors and others are considered in making a decision for the patient. There is no requirement for an advance directive for medical treatment. The goal of this advance directive is to allow someone to act as a surrogate. A  living will allows you as an individual to specify health choices for yourself and set certain restrictions in the event you become incapacitated. These will usually cover hospitalization and treatment of pre-existing conditions, but not all pre-existing conditions may be covered by a living will. You cannot designate a surrogate to make medical decisions based upon your wishes as long as you have a valid driver's license, if you are driving with the intent to injure (as determined by a law enforcement officer), are unable to communicate the wishes of the patient, or the patient is unable to communicate his or her own wishes or wishes in writing. Your living will may serve as a living will, but it shall not constitute a  declaration or expression of power of attorney or any other legal document in the place of a directive.

Hello, I am Dr. Ira Byock, one of the physicians practicing here at Dartmouth-Hitchcock Medical Center. I want to spend a few minutes talking about advance directives and why we encourage all of our adult patients to have an advance directive document on file within their medical records. I want to explain what advance directives are and why they are important to you and your family. Along the way, I hope to dispel misconceptions some people have about advance directives. Advance directives concern how people want decisions about their medical treatment to be made if they have a life-threatening condition, who people want to speak for them, and what treatments they would want to have or not have. Sometimes, when doctors, nurses, or social workers bring up the subject of advance directives, patients or their families become worried that it means their condition is serious and that we know something they do not. Let me assure you, this is not the case. At Dartmouth-Hitchcock Medical Center, we bring up the subject of advance directives with all of our adult patients. This is why we have made this video presentation. At Dartmouth-Hitchcock Medical Center, we strive to give the very best care possible every day. We save lives of people with serious illness and, whenever possible, restore people to full health and function. But while we strive to save lives and maximize people's independence and function, we also recognize that every human life eventually comes to a natural end. Therefore, whether or not a cure is possible, we strive to ensure that people's comfort is preserved and to improve their quality of life. Whenever someone is seriously ill, the people who love and care about that person, their close friends or relatives, are always affected by their condition. They feel the pains of...
